Address and old I797

Hello, 

 

I got my H1B approval from a firm A in Oct 2015 and than moved to company B which did my H1B transfer in Feb 2016. Now I am planning to go to India for stamping and have couple of questions where I hope you guys can help me 

 

1) I dint receive my original I797 from company A, just have a scan copy. Is old I797 required during interview.I have he original I 797 from company B.

 

2) For my address in passport in India, we have moved to a different place. for my H1B application, I gave same address as in passport. So should I put the same address while filing DS-160 or put new address where we have moved. Both addresses are in same city, different locality though.

 

Would really appreciate your help in this.
