Differenece between Visa Denial and Visa Refusal

While in the US on B2, I had applied for change of status from B2 to F1, which was denied but my visa was not cancelled.

Also, in the past my visa has never been refused or cancelled. I am now applying for F1 again from Pakistan. In the DS-160 application, there is a question

 

Have you ever been refused a U.S. Visa, or been refused admission to the United States, or withdrawn your application for admission at the port of entry?

 

How should I answer it?

Since you have never been refused a visa or been refused admission or have your application for admission withdrawn your answer is NO. If asked about anything be totally factual.
