DS-160 stamping question

Hello,

I have question regarding DS-160 "Have you ever been issued a U.S. Visa?"

I am at present on 4th year of H1B visa. And this is first time I am going for stamping. i.e. I had not got my visa stamped in first 3 years of H1B term. Before H1, i was F1 student visa which was stamped in passport along with Visa #.

Should I mention "Date Last Visa Was Issued" to be 10/06/2009 (start of H1B 1st 3 years term) and check option Visa # do not know?

Or mention F1 visa with visa #?

Please advise.

You should mention your F-1 visa details. He is asking for previous US visa, be it H-1 or F-1.
