DS 160 - employment dates for 221g case

hi

i worked for company A on h1b with following details

May 2011 to July 2011 - worked for Company A from US and got regular paychecks

August 2011 - came to India and went for stamping, visa was refused

October 2011 to December 2011 - client allowed to work from offshore, got one paycheck in US and then 2 checks in India, for checks in India taxes were not deducted

please guide me around employment dates to use in DS-160 when i go for stamping now.

i am planning to use employment dates as May 2011 to July 2011 for company A as i am not sure whether work done from offshore can be shown as employment, please guide me
