H4 to H1 CoS approved, can transfer h1 while in india?

My wife and I came to USA in April 2011 with me on h1 and she on h4.

We applied for her cos to h1 this year and have got her petition approved for 3 years.

She had to go back to india suddenly before in sept (before oct 1st)

My question is that she is in India and did not start working yet in usa:

Can we do a H1b transfer to another employer now while she is in india and then she goes for her h1 stamping.

Or can she travel back on her old valid h4 visa and then do a transfer of employer here?

Kindly suggest the best and easiest way so that her h1b permit is not lost.

Would really appreciate if you can help me answer these questons.

Why is the visa such a problem? Why not the original employer?

Thanks for your response, but the cleint who originally offered the job through the old employer - was not ready to wait long enough and due to the emergency it is not valid now. So the employer told that it is not possible to go for the stamping without the client letter and now that is not valid. So what is the best possible solution now considering her end client has gone.
