COS filing from H1 to H4

Hi

I am on H1 and my wife is in H4 currently. This year my wife's H1 got sponsored. If everything is positive and she is working from Oct 1st, 2012, then it is fine. However, in the worst case scenario, if she is unable to work for some reason, then how much time does she have to change the status back to H4? Does it require any pay stubs for applying Change of Status? Can you please give me an approximate fees and an Attorney who can file her COS?

Please be a little more specific . All she needs to do is leave and come back with the H4 visa in order to get back into H4 status.
