H1b visa expired

Hi

My h1b approved in 2009 and i got my visa stamped,but due to my personal issues i did not come to US at that time ,my visa expired in 2011,do i need H1b quota to apply for H1 or there will be any process like H1b extension.

If yes,then who can file my H1b petition,I mean old employer or can any company can file H1b.
