Travel outside US on approved GC

I'd appreciate if someone can help me with this info. I have a recently approved GC and planning to travel outside US soon. What all documents would I need to carry for Port of Entry when I come back to US?

I have small question,

I got green card on 02/16/2012. went out side USA on 02/20/2012. return to USA on 07/11/2012(5 months).

Again i want to go out side USA on 08/10/2012 for 11 months trip (until 07/102012) with out re entry permit.

is it problem for reentery to USA?

please advise.

Why is applying for a Reentry permit a problem? It is safer to do so for trips above 6 months.

I just read an interesting article (today) on temporary stays outside the U.S. on permanent residency (LPR) status and their potential negative impact -- http://www.******.shtm. There have been a few posts recently on this topic. The clear advice is do not make it a practice or regular occurrence, plus there are no guarantees with a re-entry permit.
