I-765 EAD for L2 Dependent

Hi All,

I am filing for 485 along with my spouse. She is on L2 and got her EAD in AUG 2011.

While filing 485 for spouse one of the form ( I-765 ) asks

" Have you ever applied for employee authorization with USCIS?"

I am not sure what answer I should write for this? Yes or No

It is also asking about documentation for "Granted or Denied " if i am saying YES what documents I need to provide for this?

Yes. Attach a copy of EAD receipt and any approval along with a copy of the EAD.
