i94 Date gaps

We have applied H4 for my wife and it got approved. She was on h1B before which was valid until sep30 2011.

Even though we applied the h4 in may 2011, it was approved recently on nov 8th with COS

My question is - on the h4 approval notice, the i94 validity from date is mentioned as nov 8th 2011.

So technically there is gap from Oct1 2011 - Nov 7 2011in the i94 dates

Is this normal or USCIS made a mistake?

The fact that an I94 was issued means she is considered to have been in status all along. It is not a mistake.

This should not be an issue since the approval notice that came with the I-94 card should indicate that the date the application was received (i.e., in May 2011), which means the case was timely filed.
