H4 to H1...Urgent

I entered US in 2009 on H4 and applied for H1 and also got it approved. From Oct1st 2009 to Jan 2010 I was on H1 but never worked in US during that period. I went back to India. Again came back to US in Jan 2011 on H4. Now I want to change my status from H4 to H1. Its on urgent basis.

What are options available for this?

1> Can I start working on H1 once I filed Change of status from H4 to H1?

2> Or Can I start working on H1 once I got the receipt of Change of status from H4 to H1?

3> Do I have to wait until I receive my Change of status get approved?

4> How many days it takes to get approve?

5> If I file premium then how much time it will take?

6> What is the fee amount for H4 to H1 in normal and premium?

Please help me, its urgent. I am in a difficult situation, I need suggestion.
