My friend is a US Citizen and staying in India over 9 months due to some health issues and taking some treatment. 

 

How long the US Citizen can stay outside the US and return to US without any issues in this case?

Forever. There is never any issue with coming to the US for a US citizen. You can stay abroad for 80 years and then come back any time. Of course, whether you can stay in or leave the other country is a separate question.

There is no requirement for a US citizen to live in the US. So, he can stay out forever.

However, a US citizen, regardless of where the person lives, has to file US tax returns, and declare the worldwide income. Usually, there are agreements with other countries to avoid double taxation, but check with a tax expert.
