H1B: How to apply H4 EAD - H4 Extension - Have valid i94 But No Valid i797

Company A filled h1 extension but did not file H4 extension in 2021
Company B filed H1 and H4 got approval copy.
H1 holder was within the county but H4 was out side country during the extension process. During entry to USA, used company B H4-i797-C copy at port of entry and extended i94.
Entered US with past visa valid till 2021, no latest visa on passport.

We realized after 2 years that i797 copy was not from company A but company B and I never joined Company B.
I am still working with company A recently 140 approved, applying EAD for spouse.
I don’t have 797 from company A.
How can I file EAD.? What are my option, Currently filing H1+H4+EAD?

1. The H4 document has nothing to do with coming back. She shows her H4 visa stamp and your H1 petition approval.

If she used the Company B H1 petition to reenter, talk to a lawyer.
