How to prepare yourself for PhD in USA (Eligibility, Credit Transfer)?

Hi,
I have done my post graduation from IIITB. I have 6 years of work experience in SAP Labs Bangalore. Last year I moved to USA on H4 visa. I am planning to do phD in USA with Stipend.
My Questions are:
1)Is course fee required as is in MS course?
2)Is GRE the only exam (apart from interviews)required to be taken?
3)Is it mandatory to get LOR from college or is it ok if we get it from the office where I worked?

4)Do we get stipend for phd?

Universities require tuition. That is not per course. It always is tuition US universities, except in community colleges (and shady institutions.) And a PhD is not about courses, anyway. For a PhD you do research, and publish about it.

Tuition is per quarter/semester.

Universities generally require the TOEFL test and GRE (some top universities, like MIT, have their own test instead of the GRE for certain programs.)

And btw, admission committees appreciate if you actually are prepared. That also is research. As pontevecchio mentioned, there is tons of information out there.
