Peculiar case: Valid i94 until 2014, recieved another i94 later until 2012

I think I have a very peculiar case

Below is order of events occurred

OLD H1 with Employer E1

Filed: 9/30/2010

Approved 10/20/2010 with i94 until 9/30/2011

Amendment 11/10/2010 filed to extend it more

NEW H1 with same Employer E1

Filed 5/30/2011

Approved on 6/15/2011 with i94 until 9/30/2014

OLD H1 amendment

Approved on 8/25/2011 with i94 until 9/30/2012

Which i94 I should go with?

Do I need another i94 after 9/30/2012

How can I have it corrected to 9/30/2014

last action rule applies, your most recent approval notice is your current status (in your case all your petitions were H1) and the attached I-94 on your most recent approval notice is your validity of stay.
