H1b Visa stamping with incorrect name issue

Recently my visa status changed from F1 to H1-B. In December I am planning to get the H1-B visa stamping done. For that process I started filling out the DS-160 form where I am having problem in the personal information. I have described the details of my concern:

I don’t have a last name mentioned in my passport. My name on passport is
Given Name: XYZ
Surname: (Blank)

In my F1- visa following naming convention was used:
Given Name: FNU
Last Name: XYZ

My H1-B petition was filed by my employer and was approved by USCIS. On the H1-B petition application and I-797A following naming convention is used:
Given Name: XYZ
Surname: LNU

I am little confused what naming convention should I exactly use in the personal information section of DS-160?
a) First Name: XYZ
Last Name: LNU

or

b) First Name: FNU
Last Name: XYZ

I will greatly appreciate your response.

Should be based on passport and H-1 B document.
