Parent's nationality by birth but migrated later

My dad was born in country A, but migrated to country B when he was 5yrs old and has been living since in country B. He has a passport of country B as well and never had passport for country A. On DS 160 there are following questions:

 

1. Country/Region of Origin (nationality) - This will be country B, am I right?

 

2. Do you hold or have you held any nationality other than the one indicated above on nationality  - If I say yes then next question is if you held passport of another country - So do I say Yes for country A nationality and then No for country A passport?  or do I say No in the first question itself because he was just 5yrs old and never had a passport of country A?
