extended review after upgraded to premium

Does any one know how long will it take to decision I140 when the I140 is in extended review after upgrading it to premium.

I have read the post where the USCIS said the application is in extended review and then when the application is upgraded to premium then it got approved. However in my case I upgraded my application to premium processing > waited 2 months, didnt hear back from them for 2 months > Asked the attorney to followup > USCIS said extended review> requested for refund (attorney suggested to request refund then they might tell us where the application may be) > USCIS replied as the refund will be processed after adjudication is complete. it is in extended review.

Waiting is pain. What can I do. Raised the request from online portal > Case status inquiry. Suppose to get back by Nov 20. not sure if they will ever get back. didnt hear back anything.
