Concurrent filing - Medical Exam

Hello All,

 

We are about to Concurrent file our I 140 and I 485. I have below questions. 

 

1) As we do not have any immunization records (I assume this is the case for many Indian applicants) from the childhood, what are the recommended immunizations shots that we need to get which are age specific and holds good for GC Medical exam.

 

2) Can anyone suggest reliable Civil Surgeons in Chicago area preferably NW suburbs (Buffalo Grove/Lincolnshire/Des Plains).
