H4 EAD application question 11

Hi, I have a question about the H4 EAD application. I came on F1 worked on OPT(I had EAD for that), then had H1 and later H4. Now while filling up the form I-765 they have a question

 

Have you ever before applied for employment authorization from USCIS?

No (Proceed to Question 12.)

 

Yes (Complete the following questions.) Which USCIS Office? Dates Results (Granted or Denied - attach all documentation)

 

ALso If I say yes (since I had OPT EAD) what dates should I enter...the start date or the start and expiry both?

The OPT EAD was work authorization.
