Questions of J-1 Waiver procedure

Hi,

I have 2 questions and would appreciate the help.

1. I am confused if I need a "Statement of Reason" with my J1 visa waiver application documents. There is no mention of it here http://travel.state.gov/content/visas/english/study-exchange/student/residency-waiver/ds-3035-instructions.html. However, some other webpages talk about it.

2. What size envelopes do I include? I am getting different sizes when I look-up legal size on google. Please let me know the size required or used by you.

1. A statement of reason is absolutely required. If you click the "STEP 1> J visa waiver online" you will find it.

2. Standard size office/letter envelope. DOS/USCIS is not going to come after you for sending the wrong size.
