H1 renewal stamping after job change - DS 160 receipt number

Hello,

 

I was on H1 visa with Employer A with expiration of Sept 30 2014. I got my H1 extension in Aug 2014 from employer A. 

 

I changed job in November 2014 and my H1 transfer was completed by Employer B in Nov. 

 

Now, I am going to India and need get my visa stamped. My question is, while filling DS 160, should I specify the receipt numbers from my Employer A for the extension as well as the receipt number from Employer B for the transfer ?

 

The reason I ask this is, my previous visa stamp expired on Sep 30 2014. Since, I had the extension from Emp A done in Aug '14 and the visa transfer from Emp B was in Nov '14, would I need to specify the receipt number for the extension from Emp A which will show my legal status for Oct '2014 - Nov 2014 ?
