Birth Certificate questions

Hi, Could someone please answer questions I have on my Birth certificate for 485 filing.

 

1. My Birth certificate was not registered during my birth. I got it from authorities recently. Do I need to notarize this or/and submit any further supporting documentations along with this Birth certificate (BC)?

 

2. My Mother's name is not mentioned this BC. Only Father's name is mentioned. Is this going to be an issue. Can I submit a affidavit from my Mother along with the BC to overcome this issue? If yes, then should this affidavit be notarized?

 

3. And are there any other documents that I need to submit along with BC having above mentioned issues?
