I-765 for OPT application

I need help with one of the entries in the I-765 application. I came to US on H4 and joined a university for master's. Since I am planning to graduate by May'15, I did a COS to F1 (which got approved). 

 

I am now getting ready to apply for OPT and one of the entries on I-765 is "Manner of Last Entry". I entered US on H4 but later converted to F1. So, what am I supposed to fill in here? Should I say H-4 or F1?
