DS3035:Some query to fill-up the form

Hi,

 

I'm going to submit the DS3035 online form and found the old pdf version while searching. I'm wondering what should I answer for the following questions below:

 

6. Did your exchange visitor program(s) include U.S. Government funds, funds from your own government or funds from an international 

organization?

 

My employer is paying me through NIH grant

 

12. List all exchange visitor programs in which you participated, beginning with the first program

Begin Date

SEVIS Number Program Number Purpose of the Form Subject/Field Code Funding Amount

 

"I'm on J1-visa. What should be the program no., Purpose of the Form, Subject/Field Code, Funding amount.

 

18. Alien Registration Number, if any

 

 

Also, is the I94 no. is the latest one, which is in my passport. Someone told me that I need to enclose my first I94.

 

Also, just wanted to make sure that I need to FedEx the documents to Waiver Review Division first along with the case no and required documents and then to the Indian Embassy.
