DS-160 Form - Employer information

In my H1B + H1B Extention application,I forget to mention one employer (for whom I worked on OPT for 15 days and have W2 from him)

(1) Is there any way I can update my H1B application information.

 

(2) How shall I fill my DS-160 form?
