Do we need birth certificate of both Indian parents when applying for PIO card for the baby?

Hi, we are the parents of 2 months old baby who is born in USA and has US passport. We (parents) are Indian citizens with H1 and H4 visas. While applying for PIO card for the baby, do we need to provide birth certificate of both parents, apart from our passport copies. My husband has his birth certificate and i do not have the same. Could you please clarify.

I believe only passport of Indian parents are required.
