Request for a copy of the final waiver certificate from CGI

Hi,

I am an Indian citizen currently residing in Toronto, Canada. I am in the stage III of the J1-visa NORI process and I need a specimen/sample of the waiver certificate that is typically issued to the applicant at the end of this stage. The J1-visa waiver process is very new for CGI-Toronto and they have NEVER issued a waiver certificate before. They have asked me to bring a specimen/sample of the waiver certificate in person.

I request anyone on this forum who might have a copy of their waiver certificate to kindly email me a copy. I promise complete confidentiality of the document.
