Form DS 160 : Name change Question

Hi,

 

I am filing Form DS 160 for my H 1 B visa. In past, I have changed my name on my passport from 

 

Last name: Blank

First name: Mintu

 

to 

 

Last name: Singh

First name: Mintu

 

In the DS-160 "Have you ever used other names (i.e., maiden, religious, professional, alias, etc.)?"  should I fill it YES or NO.

And if Yes what should I fill in  other surname and given name field ? 

 

Should it be 

 

Last Name: Mintu

First Name : FNU

 

Also what documents should I carry for visa  interview as proof of name change.
