Immigration record name correction

Hello,

I went to apply for renewal of my license and was informed about name correction in immigration record,

I have taken an info pass appointment for her name correction but want to know if there is anything else that needs to be done for this. Specifically the document that I need to carry because the web site talks about getting signature notarized and I took a print out of the form G-639.
