Regarding I-20 for COS from H-4 to F-1

Hi, I have a couple of question regd I-20 for cos

 

1. I-20 item 5 mention 2 dates a start date and a complete date. For me the start date was 01/06/2014

     so does that mean I cant apply the change of status now? and if so, do I need to reapply for a new I-20

 

2. Do i have to submit the exact same bank statement to uscis that i submitted to college
