EAD/AP Card Lost

Hello, 
My wife's EAD/AP card got lost in USPS logistics, The tracking number USCIS provided says the mail has been delivered but we never received it and i have no luck though i reached the USPS physically enquiring about it and calling USCIS customer care. Also it says the card has been picked up at Korbin, KY. not sure if that is correct location for USPS to pick up the cards for delivery. The only option i am left with is applying for EAD replacement card. 

In this regard may i please know the procedure involved, i guess i don't need to pay any fees but do i need to submit any other documents along with I-765 form and I-765/I-485 receipt notice copies.
