H4 visa, is OPT card required?

Hi,

 

I have a copy of my OPT card but not my original card (i lost it). I am changing my status from F1 (OPT) to H4 (stamping in lndia)

 

Will the consulate ask for original OPT card or will a copy do?

 

What should I do since I lost my card?
