I am traveling alone to India in a week and need to stamp my H4. My husband's H1 extension was recently approved but he will not be with me in India for stamping.

 

While scheduling my appointment the 

https://cgifederal.secure.force.com/

 

website asked for 

------principal applicant (=husband?)

 -----petitiner (=hubby's employer?)

-----start and end date (=hubby's date on 797?)

 

Also, DS 160 Q:

I was on F1 until late this year and so I have a SSN but I also have a tax identification number (when I was on F2 and didn't have SSN), do I fill out my tax id o as well or once I have my SSN filled out, it doesn't matter?
