Filing I-539 change of status from H1B to H4

Hi All,

 

My last day with my current company is 27 Dec 13. I would like to change my status to H4 (spouse is on H1b) before I could find a new job.

I thought I would file my self a paper I-539 for a change of status to H4 but some of the questions I was not able to understand, please help me with the following,

 

What should I select answers for Part 3 section of I-539,

 

1) I/we request that my/our current or requested status be extended untill?

2) is this application based on an extension or change of status already granted to your spouse,child or parent? NO, YES.... USCIS receipt no#?

3)is this application based on a separate petition or application to give your spouse,child or parent an extension or change of status? NO, YES, filed with this I-539.

YES, filed previously and pending with USCIS receipt#
