NAC and Affidavits

I do not have my Birth Certificate and about to apply for NAC and to prepare affidavits. I have my last name in abbreviated format in school certificate (Anil MJ) and in passport, visa and US driving license have the expanded format (First Name: Anil and Last Name: Meleth John). My concern is, in NAC and affidavit I should go with Anil MJ or Anil Meleth John?

School certificate having short form is normal since we in India did not have the concept of FN / MN / LN for a long time and the school certificates did not have such big column(s) for all these stuff. I would go with full name.
