Missing Last Name for Dependent H4

My spouse dont use to write last name and she have last name blank in passport. 

for example : FIRST NAME : xxxxxx  and LAST NAME :     

And i have got her VISA stamped last year , filed through my employer and when Visa came in it have :

FIRST NAME :FNU and  LAST NAME: xxxxxx

Even the I-94 have the details as :

FIRST NAME :FNU and LAST NAME: xxxxxx

 

Now the problem is , that my VISA is about to expire and i have to file an extension. And for H4 extension i need to submit G-28 and I-539 forms. My question is , what name details should i put in these forms?

Should i go with name as in Passport: FIRST NAME : xxxxxx and LAST NAME :

OR

Should i go with name as in VISA/I-94 : FIRST NAME :FNU and LAST NAME :xxxxxx

For the new extension do not put fnu. Put the original name . It will pass
