On H4 visa- H1B approval pending

Hi,

I arrived in the US in June 2013 while my H1 application was pending.

Once the H1B was filed and picked in the lottery, I applied for a H4 visa and arrived in the US.

Now my question is if and when my H1 is approved, can I start working immediately or do I have to leave the country and again get my H1B stamped? Or can I just file a Change of Status so that I don't have to go back to India?

Either you can leave the country and enter with a H1B visa or employer has to file for COS and pay H1B fees. I do not think your employer would be interested to spend H1B fees again.
