Name discrepancies in approval notice and passport.

I had my h4 visa renewed by the USCIS in my maiden name.

during my india trip , i got a new passport with my spouse name added and hence my surname has changed.

i need to go for a stamping now and not sure if this will be an issue(my old passport is cancelled).

please advice.

i have got a notarized "one and the same person affidavit". is there anything else i need to carry?

and in wht name will i get the new visa stamped?
