My employer just filed for extension and filed premium so should receive the approval soon. Now im planning to visit india next month. What is the process to change my wife's appointment to a couples appointment so we both can get the stamping while im ther.

You need to fill another DS160 for yourself and schedule an appointment at the same time like hers.

Just take both the petitions and show them to the first interview session, they will update it in their system with the new petition number. Technically you i94 will be the same in both the petitions and the US consulate offer can see that it has been extended
