I-94 Confusion.

We have one I-94(First) while we entered in USA.

and then Extended same status got another I-94 (second)

 

Then status change got another I-94.(Third)

 

now while leaving the country. ..

 

we have to provide ALL I-94's or 

 

First and Third OR only Third.
