Expired H1B extension

Situation:

 

Currently working in US, on extension of L1B VISA - from employer A (Expected reply date form USCIS is March 2014).

 

Have an approved petition of H1B for the same employer A which expired on 20-May-2013.

 

Questions:

 

1) Being on visa extension as of date, can I get the expired H1B visa extended by Employer A While I am in the USA?

 

2) Being on visa extension as of date, can I get the expired H1B visa extended by any other employer ,say Employer B in the USA?

 

3) If I file for extension of H1B from the same employer A from India, will I have to wait till April 2014 to apply for it and start working on it only after October 2014?

 

4) Being on visa extension as of date, can I apply for H4 dependent VISA ?
