Existing H1B + new H1B

I have an H1B from employer X. It is valid till 2015. However employer X has never sent me to US(H1B is never used).
A new employer Y wants to hire me in US.
1) Does employer Y have to file for a completely new one ?

2) Does it take 4-6 months (again) for processing ?

3) Does employer Y have to invest $ 5000-6000 again?
4) Is there a possiblility of rejection, if employer Y is a smaller firm ?

(2) It would take a minimum of 2.5 months by regular processing and 15 calendar days by premium processing.

(3) Yes, same H1B fee applies

(4) Rejection does not depend on size of firm but whether the employer and employee have good credentials.
