(section F). 3. Monthly pay (shall vary per pay stub) based on bonus, overtime, etc... 4. annual income menitoned in the offer letter/12. any help is much much appreciated. Link to comment

I believe it should be your monthly income, as simple as that. Your monthly income is the minimum constant amount that your employer pays you. Bonus, overtime are just extra variable pay, which you get on top of your salary. Link to comment
