However, the name on his Certificate of Naturalization omitted his middle name because he did not put down his middle name when he filled out and signed N-649 (Oath Declaration Sheet ) and N-400 (Application for Naturalization). After he received his citizenship certificate, he realized the discrepancy between the Citizenship certificate and his Green Card/Social security card/indian passport etc. He filed Form N565 (Application for Replacement of Naturalization Citizenship Documentation) to have the citizenship certificate corrected/reissued with his full name. He supplied supporting documentation (CA Driver's license, U.S. Social Security card, a copy of his Indian passport, a copy of his Green Card). USCIS denied his request and asked him to file Form I-290B Notice of Appeal or Motion with supporting documentation. The stated reason was that when he filled out the N-649 and N-400, he did not put down his middle name, and the Certificate of Naturalization he was issued was based on these two forms. What additional documentation should he provide to make sure his that his Naturalization Certificate show his full name? Without his full name, his US passport will not match his Drivers license and Social security card; he also cannot get his Overseas Citizen of India (OCI) card and will have trouble with legal paperwork in India as well as the United States. Please advise as soon as possible as he has until July 20 to respond. Link to comment
JoeF Posted July 17, 2013 Report Share Posted July 17, 2013 It is his fault if he didn't provide that information on the N-400. He would have to do a name change. This is a somewhat complex process, with the district court. Link to comment
