Help to fill DS160 - Have you traveled to any countries within the last five years?

Hi Friends, 

 

I am kind of confused with this question as I traveled to India and back to USA till now from past 5 years.

And also I am going to Canada for stamping next month , 

 

Do I have to mention USA,INDIA and CANADA in DS 160 form ?
