Documents required for Question.11 in i-765 form for STEM Extension (OPT Ext.)

Hello Guys,

I am currently on my OPT visa which is valid until June 3rd. I am currently in the process of applying for STEM Extension, and I am a bit confused on Q.11 in the i-765 Form which needs to be filled. The question reads:

Have you ever applied for employment authorization? If Yes, then which USCIS office, Date(s) and Results (Granted or denied, attach all documents)

What does Attach all Documents mean?

The reason I ask this is because, I am wondering if they are expecting us to also provide them with the "mailer" that came with my OPT card? or Any other letter that came before the card arrived, like (i-797,i-797C...)

As of this time, I do not have the mailer, but only my EAD card and the initial i-797 which is Notice of Action stating that they received my OPT request on Mar 14th.

Please help as to if I will be in trouble without providing the mailer that came with my OPT card the first time..
