COS to h1b Stamping in India after 3 months

Hi,

I was on COS from l1 to H1b visa and I was working with "X" company and they deputed me in "Y" company. I got laid off from "Y" company and after that my company gave me an offer to work Inhouse on half salary so that meanwhile thay can find another project for me. so i worked there for 2 months on half salary. but my company was unable to find out any project for me so they offer me to come back to India and work in India office.i came to India on 29 Dec 2012.

i came here and applied for new passport as my passport was damage and about to expire. I did it in Jan 2013 and got my passport in Feb 15th.

after that i joined my India office and now i want to apply for stamping. I have below doubt.

1) do i need new offer letter from US office.

2) do i need Indian salary slip?

3) what are the other documents i need?
