H1B renewal for expired I797 ?

Hello,

I got through cap rounds and received I-797(b) in 2007 alongwith I-129 from my US employer.

I did not go for H1B stamping that time. Because of personal reasons I did not move to US.

I-797 was valid till Sept 2010.

Considering H1B can be renewed once (6yrs timeline) and that I have already been through the cap round once; If I apply for a US job now, will I have to go through the CAP round again ?

Or renewal/extention of H1B can happen ?

Or Do I have to reapply now ?
