third party barcode page in DS-3015

Hi,

I have just started my J-1 waiver process and i have a minor question regarding this. Do one needs to attach any documents to the third party barcode page in DS-3015 application while sending to Washington embassy or is it just that one page is enough to send.

You do not have to send anything to Washington embassy. Have a look at this website. It explains clearly how one has to proceed to get a J1 waiver
